Information Needed to Request a Transcript

In order to provide you with your official transcript as expeditiously as possible, please provide the following information when you make your request. For your convenience, it is suggested that you complete our Official Transcript Request Form to insure that all required information is provided. To fill in this form online, place the text tool in a field and type. Print the completed form to add the required signature.

  1. Full name and any former names
  2. Original signature
  3. Social Security number
  4. Date of last attendance
  5. List each type of transcript(s) requested: (undergraduate, graduate)
  6. Number of copies
  7. Name and address where transcript(s) are to be mailed
  8. Your daytime telephone number

Requests are processed in the order received. Normal processing time is five business days after receipt of the request. However, there are times within each semester when individual requests may be delayed due to University Policies and Procedures that must be followed to ensure the accuracy of all academic records. We do not notify requestor of either receipt or completion of their request.  If the student has a hold on their account the University will NOT process the request, instead a letter will be issued to the requesting party indicating why the request cannot be fulfilled.

Ordering by Fax:

The fax machine receiving transcript requests is connected to a dedicated telephone line and is located in the transcript department, a limited access area.

You must have access to your MyLeo account if you wish to take advantage of this convenient ordering process. If you have forgotten your MyLeo account information your may contact Technology Services at 903-468-6000

To order your official or unofficial transcripts follow these easy steps:

  1. Log on to your MyLeo account
  2. Go to Registration, Record, Grades; this is bring up a new page
  3. On the new page, click Student Records; This will bring up a new page with all of your student records
  4. Click Request Printed Transcript
  5. This will bring you to our online request system. Follow the directions and be sure to check the final screen to ensure that your transcript will be mailed to the proper location

Ordering In Person:

Current and former students may order official transcripts in person from the Registrar's Office, located in the Student Access and Success Center at 2200 Campbell Street. Office hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. A picture ID is required when ordering a transcript. Normal processing time is five business days after receipt of the request. There are times within each semester when individual requests may be delayed due to University Policies and Procedures that must be followed to ensure the accuracy of all academic records.
