Below you will find safety programs, training material, information and tools to use for your environmental health and safety needs.
Travel Risk Assessments
Travel Risk Assessment Only: For travel that does NOT include reserving transportation or use of vehicles from the Transportation office. Please allow 7-10 days for processing and approvals.
Transportation Request: For requesting transportation or use of vehicles from the Transportation office and includes the Travel Risk Assessment. Please allow 7-10 days for processing and approvals.
Driver Certification Application: Only Texas A&M University-Commerce employees may operate a University vehicle or vehicles rented by the University through commercial agencies and must be on the list of approved drivers. This list is maintained by the Transportation office and this form is to request that a driver is approved and on file.
Waiver, Indemnification, and Medical Treatment Authorization Forms
An undergraduate, graduate, or professional student who participates in travel related to academic course requirements (e.g. field trips, study abroad), or their assigned duties as a University employee, (e.g. research data collection) shall not be required to sign a waiver or release in relation to that travel.
- Students voluntarily participating in elective activities and programs requiring travel (e.g. student organization travel, international travel (non- credit bearing), optional academic field trips, events) will be required to complete a waiver and release form verifying that they understand and accept the risks involved in participating in the travel and/or activity, and assume responsibility for their behavior. Students under the age of eighteen (18) must have a release form signed by their parents or legal guardian.
Event Risk Assessments
Event Risk Assessment Only: This form is for events that are in spaces NOT reserved through the online Reservation Request form. Please allow 2 weeks for processing and approvals.
Reservation Request: This form is used to request reservations in the RSC, MRC and various other buildings and green spaces on campus and includes the Risk Assessment. Please allow 2 weeks for processing and approvals.
Authorization for Outdoor Grilling/Burning: This form is to be completed prior to any outdoor grilling/burning taking place and is used to schedule Fire Extinguisher training and to obtain a fire extinguisher if needed. Please allow 7-10 days for scheduling.
